Really Big Shoe V Photo Order Form

Please fill in the order form following the steps below:

1) View the photo gallery at the following URL:
   http://www.trilliumacademy.com/ReallyBigShoeV_2005/index.html

2) Write your name, phone number and address in the space provided.
   
3) Neatly print the names of the pictures along with the quantities for each 
   size. You can also enter a description for cross reference.

4) Add the total number of prints for each size in the space at the bottom of 
   the page.  Enter the total on the "Total" line.
   
5) Drop your off your order form and payment (cash or cheques) 
   with Kathleen or Patty.

NOTE: If you require more than 10 different pictures you can staple multiple
      order forms together with a total on the last page.
      (copies of this form are available on the web site).
   
Please complete your order and drop it off by May 14, 2005 the orders should 
be filled within about 2 weeks of the closing date.
